Participant Publicity Release

The North American Mission Board of the Southern Baptist Convention, Inc. and Send Relief, Inc. and Center City Church ("NAMB/Send Relief/CCC") desires to engage in certain promotional activities with above Participant in furtherance of NAMB/Send Relief/CCC's Christian ministry programs, and Participant desires to voluntarily participate in such promotional activities.

Permission. Participant authorizes and permits NAMB/Send Relief/CCC to display, transmit, broadcast, record, photograph, digitize, modify, edit, or otherwise use Participant’s name, image, likeness, appearance, voice, biographical information, and all materials created by or for NAMB/Send Relief/CCC that incorporate any of the foregoing (collectively, the “Materials”) in any time or place and in any medium or format.

Ownership. NAMB/Send Relief/CCC will be the exclusive owner of any created Materials and all related rights, without payment or royalty. NAMB/Send Relief/CCC may, but is not required to, permit Participant to preview certain Materials before use by NAMB/Send Relief/CCC. NAMB/Send Relief/CCC has no liability for any editing or alteration of the Materials, use of the Materials, or presentation of Participant. NAMB/Send Relief/CCC has no obligation to create or use the Materials.

Liability Waiver and Release. Participant waives and releases all legal and equitable rights, liabilities, claims, demands, actions, suits, damages, and expenses, including but not limited to claims for copyright or trademark infringement, infringement of moral rights, libel, defamation, invasion of any rights of privacy (including intrusion, false light, public disclosure of private facts, and misappropriation of name or likeness), violation of rights of publicity, physical or emotional injury or distress, or any similar claim or cause of action in tort, contract, or any other legal theory, now or hereafter known in any jurisdiction throughout the world (each a “Claim” and collectively, "Claims"), arising directly or indirectly from the rights or activities described in this Participant Publicity Release (“Release”), whether resulting in whole or in part from any negligence of NAMB/Send Relief/CCC or any other person, against NAMB/Send Relief/CCC or any of its employees, contractors, agents, representatives, affiliated entities, successors or assigns (collectively, “Released Parties”). Participant, on behalf of Participant and his or her heirs, representatives, successors and assigns, forever releases and discharges the Released Parties from liability under such Claims. Participant understands NAMB/Send Relief/CCC is relying on this Release and will incur significant expense in reliance on this Release, and Participant agrees this Release cannot be terminated, rescinded, or modified.

Disputes. Any dispute concerning this Release will be resolved by mediation and, if necessary, by binding arbitration in Atlanta, Georgia under the rules of a mutually agreeable Christian Conciliation service. NAMB/Send Relief/CCC and Participant waive their rights to sue, appeal, or seek other legal remedies with respect to this Release, except to the extent necessary to enforce a mediation agreement or final arbitration award.

BY SIGNING BELOW, PARTICIPANT (OR MINOR PARTICIPANT’S PARENT OR LEGAL GUARDIAN ON BEHALF OF THEMSELVES AND MINOR PARTICIPANT) ACKNOWLEDGES HAVING READ AND UNDERSTOOD ALL TERMS OF THIS RELEASE AND AGREES TO BE BOUND BY THIS RELEASE.

Other Than The One Listed Above
I am a parent or legal guardian of the above named camper(s) who is currently under the age of 18. I do hereby release Center City Church and it's agents, including pastor, members, and volunteers from any and all liability or responsibility due to any injury that he/she may incur as the result of, or arising in any way from participation in any program, activity, event, transportation or services sponsored by Center City Church and I agree to waive any right, consent to accept these risk and voluntary agree to allow his/hers participation in activities. If I cannot be consulted in an emergency, I give permission to the physician selected by a representative of Center City Church to hospitalize, secure treatment for, and to order injections, x-rays, anesthesia, and/or surgery for the person named above. I agree to fully pay all cost incurred for the care of the minor if such emergency arises.

These authorizations shall remain effective as long as the camper participates in the programs, activities, events, or services of Center City Church unless revoked in writing by the parent or guardian signed above, and delivered to the agent(s) of Center City Church.
